The name of the person listed as "physician or person signing certificate or affidavit is Judson Tanner of Clear Lake, Wisconsin (listed as an uncle of Lloyd Rogers). Information on birth and parentage of Lloyd Rogers obtained from birth certificate #01240 (Polk County, Wisconsin, Reel#322, Record Number DX1240 [the "D" code meaning "Delayed" birth certificate] via Wisconsin State Historical Society Library). The affidavit or certificate was executed on October 8, 1938 and registered on October 17, 1938. There probably was no formal birth certificate registered at the time of Lloyd's birth, but that one was finally registered in order to facilitate registration with Social Security. Dr. Rogers was the first medical doctor to work out of the newly constructed hospital when he began there in 1900. Before obtaining his medical degree from McGill University in Montreal, he had the distinction of being one of Canada's earliest appendectomy patients. While working as a postal inspector, he again led the way, carrying the first official mail to Victoria on the newly completed Canadian Pacific Railway. He was a well-loved resident of Chemainus for many years, and retired as the Medical Superintendent of the Chemainus Hospital in 1936.
